A welfare visit to one of our tenants in Highbridge highlighted how partnership working can make a real difference.
After a fall, a tenant who had previously been active and independent experienced a significant decline in her health and confidence. Living alone, she used her Lifeline pendant to report concerns, including not having enough food at home.
An Extra Care Housing Officer from our Independence and Wellbeing Service visited the tenant and arranged an emergency food shop. Recognising that longer-term support was needed, the team worked with a local Micro-Provider and Village Agent to put additional help in place.
Over the past few months, the tenant has been supported with regular shopping trips, which has helped her eat more healthily, attend GP appointments and regain some of her independence. She has also gained the confidence to focus on her personal wellbeing, including having her first haircut in months.
This is a great example of how Homes in Somerset, Village Agents and Micro-Providers work together to help tenants stay independent, improve their wellbeing, and continue living safely in their own homes.
